dfed547529 glib: stop leaking idle and hrtimer source
This gives a valgrind result for lws-minimal-http-server-event-lib-foreign
that's clean for lws pieces, although glib is always dirty wrt thread local
storage and hashtable.

286cf4357a sul: multiple timer domains
Adapt the pt sul owner list to be an array, and define two different lists,
one that acts like before and is the default for existing users, and another
that has the ability to cooperate with systemwide suspend to restrict the
interval spent suspended so that it will wake in time for the earliest
thing on this wake-suspend sul list.

Clean the api a bit and add lws_sul_cancel() that only needs the sul as the
argument.

Add a flag for client creation info to indicate that this client connection
is important enough that, eg, validity checking it to detect silently dead
connections should go on the wake-suspend sul list.  That flag is exposed in
secure streams policy so it can be added to a streamtype with
"swake_validity": true

Deprecate out the old vhost timer stuff that predates sul.  Add a flag
LWS_WITH_DEPRECATED_THINGS in cmake so users can get it back temporarily
before it will be removed in a v4.2.

Adapt all remaining in-tree users of it to use explicit suls.

b3131fdfdd cmakelist: Augean Stables refactor
Establish a new distributed CMake architecture with CMake code related to
a source directory moving to be in the subdir in its own CMakeLists.txt.
In particular, there's now one in ./lib which calls through to ones
further down the directory tree like ./lib/plat/xxx, ./lib/roles/xxx etc.

This cuts the main CMakelists.txt from 98KB -> 33KB, about a 66% reduction,
and it's much easier to maintain sub-CMakeLists.txt that are in the same
directory as the sources they manage, and conceal all the details that that
level.

Child CMakelists.txt become responsible for:

 - include_directories() definition (this is not supported by CMake
   directly, it passes it back up via PARENT_SCOPE vars in helper
   macros)

 - Addition child CMakeLists.txt inclusion, for example toplevel ->
   role -> role subdir

 - Source file addition to the build

 - Dependent library path resolution... this is now a private thing
   in the child CMakeLists.txt, it just passes back any adaptations
   to include_directories() and the LIB_LIST without filling the
   parent namespace with the details

9f1d019352 CTest: migrate and deprecate existing selftest scripts
Replace the bash selftest plumbing with CTest.

To use the selftests, build with -DLWS_WITH_MINIMAL_EXAMPLES=1
and `CTEST_OUTPUT_ON_FAILURE=1 make test` or just
`make test`.

To disable tests that require internet access, also give
-DLWS_CTEST_INTERNET_AVAILABLE=0

Remove travis and appveyor scripts on master.

Remove travis and appveyor decals on README.md.

d3308df40f windows: pthreads
Add support for external pthreads lib on windows and some docs about how to do.

It can build with LWS_WITH_THREADPOOL and LWS_WITH_MINIMAL_EXAMPLES including the
pthreads-dependent ones without warnings or errors on windows platform as well with this.

pthreads_t can be anything, including a struct - not a pointer-to-a-struct
but the struct itself.  These can't be cast to a void * for printing as they can
on linux, where the base type is a pointer.

Let's fix all the usage of those to determine their own thread index in terms
of the meaning to the program rather than as a tid.

954fcf2bc3 windows: clean LWS_WITH_MINIMAL_EXAMPLES warnings
Fix pthreads detection in the minimal examples and add it where needed.
Fix unistd.h include to be conditional on not WIN32

With this, -DLWS_WITH_MINIMAL_EXAMPLES=1 is happy and warning-free
on windows.

8a7e0edb7d lws_spawn_piped: break out from cgi
The vfork optimized spawn, stdxxx and terminal handling in the cgi
implementation is quite mature and sophisticated, and useful for
other things unrelated to cgi.  Break it out into its own public
api under LWS_WITH_SPAWN, off by default.

Expand it so the parent wsi is optional, and the role and protocol
bindings for stdxxx pipes can be set.  Allow optional sul timeout
and external lws_dll2 owner for extant children.

Remove inline style from minimal http-server-cgi

0f7f27801e http redirect: 303: force method to GET
This teaches http client stuff how to handle 303 redirects... these
can happen after POST where the server side wants you to come back with
a GET to the Location: mentioned.

lws client will follow the redirect and force GET, this works for both
h1 and h2.  Client protocol handler has to act differently if it finds
it is connecting for the initial POST or the subsequent GET, it can
find out which by checking a new api lws_http_is_redirected_to_get(wsi)
which returns nonzero if in GET mode.

Minimal example for server form-post has a new --303 switch to enable
this behaviour there and the client post example has additions to
check lws_http_is_redirected_to_get().

f9f6bb66fe lws_validity: unified connection validity tracking
Refactor everything around ping / pong handling in ws and h2, so there
is instead a protocol-independent validity lws_sul tracking how long it
has been since the last exchange that confirms the operation of the
network connection in both directions.

Clean out periodic role callback and replace the last two role users
with discrete lws_sul for each pt.

ba754c4cb2 h2: unify immortal stream tracking across SSE and ws substreams
It was already correct but add helpers to isolate and deduplicate
processing adding and closing a generically immortal stream.

Change the default 31s h2 network connection timeout to be settable
by .keepalive_timeout if nonzero.

Add a public api allowing a client h2 stream to transition to
half-closed LOCAL (by sending a 0-byte DATA with END_STREAM) and
mark itself as immortal to create a read-only long-poll stream
if the server allows it.

Add a vhost server option flag LWS_SERVER_OPTION_VH_H2_HALF_CLOSED_LONG_POLL
which allows the vhost to treat half-closed remotes as immortal long
poll streams.

498a4e2bd7 sul: all timed objects use a single pt sul list
wsi timeout, wsi hrtimer, sequencer timeout and vh-protocol timer
all now participate on a single sorted us list.

The whole idea of polling wakes is thrown out, poll waits ignore the
timeout field and always use infinite timeouts.

Introduce a public api that can schedule its own callback from the event
loop with us resolution (usually ms is all the platform can do).

Upgrade timeouts and sequencer timeouts to also be able to use us resolution.

Introduce a prepared fakewsi in the pt, so we don't have to allocate
one on the heap when we need it.

Directly handle vh-protocol timer if LWS_MAX_SMP == 1

bd36962c17 openssl: add threading apis needed for 1.0.2 and earlier
In later OpenSSL, eg 1.1.1, these apis are NOPs in OpenSSL.
They are required for normal operation with multithreading
on earlier OpenSSL, eg, 1.0.2k

This doesn't change the intention or status of the CC0 files, they were
pure CC0 before (ie, public domain) and they are pure CC0 now.  It just
gets rid of the (C) part at the top of the dedication which may be read
to be a bit contradictory since the purpose is to make it public domain.

6d45e6c5b1 spa: add info args and stride
This is aimed at allowing a stride to optionally be
given for the parameter name array... this will allow
use of lws_struct metadata as the parameter name
array.


Also introduce the option to put all allocations in
an lwsac instead of via lws_mallocs.

0079099f10 vhost: add pprotocols to vhost info
info.protocols works okay, but it has an annoying problem... you have to know
the type for each protocol's pss at the top level of the code, so you can set
the struct lws_protocols user_data size for it.

Lws already rewrites the protocol tables for a vhost in the case of runtime
protocol plugins... this adapts that already-existing code slightly to give
a new optional way to declare the protocol array.

Everything works as before by default, but now info.protocols may be NULL and
info.pprotocols defined instead (if that's also NULL, as it will be if you
just ignore it after memsetting to 0, then it continues to fall back to the
dummy protocol handler as before).

info.pprotocols is a NULL-termined array of pointers to lws_protocol
structs.  This can be composed at the top level of your code without knowing
anything except the name of the externally-defined lws_protocol struct(s).

The minimal example http-server-dynamic is changed to use the new scheme as
an example.

986bb37c88 ah: custom headers for h1
Until now lws only parses headers it knows at build-time from its
prebuilt lexical analyzer.

This adds an on-by-default cmake option and a couple of apis
to also store and query "custom", ie, unknown-to-lws headers.

A minimal example is also provided.

At the moment it only works on h1, h2 support needs improvements
to the hpack implementation.

Since it bloats ah memory usage compared to without it if custom
headers are present, the related code and ah footprint can be
disabled with the cmake option LWS_WITH_CUSTOM_HEADERS, but it's
on by default normally.  ESP32 platform disables it.

1) update the logos to svg

2) add svg icon for strict security policy where used

3) define new vhost option flag to enforce sending CSP headers
   with the result code

4) add vhost option flag to minimal examples to
   enforce sending CSP where applicable

5) Go through all the affecting examples confirming they
   still work

6) add LWS_RECOMMENDED_MIN_HEADER_SPACE constant (currently
   2048) to clarify when we need a buffer to hold headers...
   with CSP the headers have become potentially a lot
   larger.

f6ae0edf8d strict host check vhost flag

If you vhost->options has the flag LWS_SERVER_OPTION_VHOST_UPG_STRICT_HOST_CHECK,
then if the server is sent an upgrade request, the content of the Host: header is
required to match the vhost name + port.  The port is set to the well-known values
of 80 and 443 if no :port on the host: value, depending on tls or not on the
connection.

minimal-ws-server can now take a -h flag to set this.  lejp-conf (eg, lwsws) can now take
a flag strict-host-check on the vhost to enable it as well.
