various dvb related fixes
...
- use FE_GET_EVENT to get faster status updates from dvb adapter frontend
- avoid using dvb muxes that are not locked or if they have too many FEC errors

Major rework, as follows:
...
- Add automatic DVB mux scanning
- Rename "programme" to "event" (more consistent with DVB specs)
- Improve showtime client interface
- Add decent timers
- Fix UTF-8 conversion issues at various places
- Improve EPG (Join DVB and XMLTV to same lists)
